If you need a Certificate of Affiliation from SeriousMD (e.g. for visa applications or professional purposes), you can request one directly from us. Please note that there are certain eligibility requirements.
Eligibility Requirements
To request a Certificate of Affiliation, you must meet the following criteria:
Active on the platform for at least 6 months – This ensures that you have a consistent record with SeriousMD.
Minimum of 50 patients – This demonstrates that you are actively using the platform to see patients.
How to Request
Send us a message via the platform.
Include the following information:
Full name as registered on SeriousMD
Purpose of the certificate (e.g., visa application)
Once your request is received and verified, we will prepare the Certificate of Affiliation for you.
🚦 We reserve the right to refuse the generation of a Certificate of Affiliation for any reason whatsoever.
Important Notes:
The Certificate of Affiliation does not and cannot indicate employment, as providers are not employed by SeriousMD or NowServing. It only confirms that you are an active doctor on the platform.
